Community Childcare Basics
Starshine is a premium childcare provider partnering with Citizens4Community to provide FREE childcare services during their bi-annual Community Forum on May 5, 2024 for kids ages 1+. The Forum will be held at the Sisters-Camp Sherman Fire Hall with childcare across the street in the Village Green Park. Childcare services are available only to those attending the forum, and only to those registered by noon Friday May 3rd, 2024.

1. Filll out this registration form no later than April 26th
2. Sign this liability waiver and upload on the registration form

Be Prepared!
Please make sure your child comes with:

appropriate outdoor clothing according to the weather

an extra warm layer

a water bottle (labeled with name)

durable shoes (Sneakers or hard-soled sandals, like Chacos or Keens, no flip flops)

sunscreen already applied and/or hat
